1. Application form 012/LAB ( Available at all public services centres and service centres of the Civic and Municipal Affairs Bureau (IACM) free of charge or downloadable at IACM’s website);
2. Document of application for testing services issued by a public entity.

Important notes1. The application form should indicate:
a) Name of applicant (Public entity corresponds to the name of a department);
b) Address;
c) Name of items to be tested;
d) Address, date and time for sampling;
e) Name of contact person;
f) Reasons for test.
2. No declaration for the difference in results due to different time for analyzing the tested items;
3. Each request for laboratory testing will be subject to the approval of our laboratory depending on individual circumstances.

Application feeAccording to the current charge list (the Chief Executive Order n.º319/2016, revised the Fees, Charges and Prices List of Civic and Municipal Affairs Bureau.(IACM).), published in Part 1 of Edition 36 of the Macao SAR Gazette of 5th September 2016). for details, please refer to the following Fees, Charges and Prices List.
Fees : IACM Laboratory does not accept private or private entities to apply for analysis services, prices listed in the laboratory fees only apply in the swimming pool and whirlpool water sample analysis costs, and assist public entities to conduct research and laboratory analysis costs.
