Documents to be submitted1. A completed Application Form for Cremation of Skeletal Remains;
2. A legible photocopy of identity document of the applicant.
3. Document stating the origin of the remains, and the applicant needs to apply for a Permit for Relocation of Human Remains.
Optional documents for submission (required to submit if applicable):
1. For skeletal remains from a non-public cemetery, submit a legible photocopy of the identity document(s) of the deceased.
Documents to be presentedOriginal of identity document of the applicant.
Important notes1. The applicant is obliged to bring the skeletal remains of the deceased to the Our Lady Mercy Cemetery;
2. If the applicant has not retrieved the cremains within 180 days after the cremation,the applicant will be regarded as given up his/her right to dispose of them, and IACM reserves the right to process the cremains appropriately.
Target groupThose who intend to cremate the skeletal remains of their deceased family members or relatives.
Application fee1. Cremation service:MOP300.00 (temporarily exempted)
