2. Fill in the Declaration (019/SAL);
3. Receipt or supporting documents on right of use of bone box/ash box;
4. A legible photocopy of identity document of the applicant.
Documents for optional submission (Mandatory, if applicable):
1. For bone remains or bone ashes obtained from private cemeteries, the applicants are required to apply for License for Relocation of Human Remains into Public Cemeteries and submit a legible photocopy of identity document of the deceased person. For bone remains, applicants are required to submit the License for Relocation of Human Remains issued by the Health Bureau. For bone ashes, they are required to submit supporting documents on cremation.
