2. Supporting document(s) or declaration(s) (020/SAL) to identify the relationship between the applicant and the deceased person;
3. A legible photocopy of identity document of the applicant ;
4. Declaration of the funeral and construction work(021/SAL) ;
5. Funeral Services Company has responsible for providing a legible photocopy of identity document of the contact person.
Documents for optional submission (Mandatory, if applicable):
1. Design layouts or photos illustrating the kind of construction work to be carried out in the tomb (permanent tomb);
2. Receipt for rental of the tomb or other supporting documents "Permanent Tomb";
