3. Fill in the Declaration (019/SAL) ;
4. A legible photocopy of identity document of the applicant.
Documents for optional submission (Mandatory, if applicable):
1. If the applicants require IACM to render burial service, they have to fill in the respective application form and submit it to IACM;
2. If exhumed by funeral Services Company, it is necessary to fill in the declaration of the funeral and construction work (021/SAL), and also provide the legible photocopy of identity document of the proprietor of the funeral Services Company;
3. For application of construction work permit, the applicants are required to fill in the respective application form and state out the dates of the commencement and completion of the construction work;
4. A legible photocopy of identity document of the deceased person (For Human Remains, bone remains or bone ashes obtained from public cemeteries, it is no need to submit the legible photocopy of identity document of the deceased person);
5. For bone remains or bone ashes which are not from public cemeteries, the applicants are required to process the license for relocating them into the public cemetery. As for bone remains, the applicants are required to submit the Relocation License of Human Remains issued by the Health Bureau. For bone ashes, supporting document(s) on cremation is necessary for submission;
6. Required to present the Death Certificate issued by the Civil Affairs Registry upon processing of the procedures for Joint Burial;
7. For those who died abroad other than in the Macao SAR, if the time difference between their death and the import of their dead bodies is less than 60 hours, the applicants are required to submit a Report on the Actual Transfer of Dead Bodies issued by the Public Security Police Force (Information Department / Commissariat of General Affairs / Section of Supervision and Register). If the time difference exceeds 60 hours, the applicants are required to submit a Funeral and Burial Pass.
