Documents to be submitted1. Supporting document(s) or declaration(s)(020/SAL) to identify the relationship between the applicant and the deceased person;
2. A legible photocopy of identity document of the applicant;
3. A legible photocopy of identity document of the deceased person.
Additional documents to be submitted (those that must be submitted when applicable):
1. If the applicants require IACM to render burial service, they have to fill in the respective application form and submit it to IACM;
2. If burial by funeral Services Company, it is necessary to fill in the declaration of the funeral and construction work (021/SAL), and also provide the legible photocopy of identity document of the proprietor of the funeral Services Company;
3. For application of construction work permit, the applicants are required to fill in the respective application form and state the dates of the commencement and completion of the construction work;
4. Death certificate of the deceased issued by the Civil Affairs Registry;
5. For those who died abroad other than in the Macao SAR, if the time difference between their death and the import of their dead bodies is less than 60 hours, the applicants are required to submit a Report on the Actual Transfer of Dead Bodies issued by the Public Security Police Force (Information Department / Commissariat of General Affairs / Section of Supervision and Register). If the time difference exceeds 60 hours, the applicants are required to submit a Funeral and Burial Pass.
6. For impoverished applicants who are incapable of paying the burial fee, they are required to submit relevant supporting document(s)/certificate(s).

Important notes1. Children: Stillbirths and children up to 7 years old;
2. For burial in ordinary tomb, IACM could exempt the applicants from payment of the respective fee, other than the license fee for construction work, upon their request and provision of proofs that they are financially incapable of paying.
