Documents to be submitted1. Application Form for Retail License/Import Registration of Meat (Mod. 035/SIS);
2. A legible photocopy of the identity document of the applicant (with both sides of the identity document photocopied on the same page of an A4 sized paper);
3. For application made by a legal person, the original or a photocopy of the Business Registration Certificate issued by the Commerce and Movable Property Registry with a valid period of 3 months should be submitted;
4. A photocopy of M/1 Form – Business Registration Tax – Declaration of Initiation of Activities/Alterations registered in the Financial Services Bureau;
5. The original certificate proving the property ownership of the establishment concerned issued by the Real Estate Registry recently;
6. A photocopy of the location plan of the establishment in the scale of 1:1000;
7. A photocopy of the original construction plan of the establishment;
8. The original location plan of the facilities of the establishment in the scale of 1:100 with explanatory notes;
9. A photocopy of the final construction plan approved by the Land, Public Works and Transport Bureau (DSSOPT) should be submitted. Application for intended renovation or alteration work of the establishment should be made to the DSSOPT according to its specified requirements, and a photocopy of the letter with feasible plans issued by the DSSOPT should be submitted;
10. If the establishment occupies a total area of over 400 square metres or its refrigeration room has a capacity of over 140 cubic metres, photocopies of the fire service installation plan approved and the letter with feasible plans issued by the Fire Services Bureau should be submitted;
11. The original Operation Proposal and Information Registration Form for Retail/Import Establishments (Mod. 054/DIHA/SIS).
Documents to be presentedThe original identity document must be produced.
Important notes1. After submission of all required documents, the establishment shall undergo inspection conducted by the relevant unit of the IACM;
2. Import registrations issued are non-transferable and the registered establishments must not be moved;
3. The application procedure shall only be processed upon receipt of all required documents;
4. The import registration is valid till the end of the year in which it is issued;
5. Though the import registration does not require payment of an annual license fee, applicants are required to file their applications which shall be examined and assessed to determine if they meet the necessary requirements for registration of establishments for import;
6. According to the existing regulations governing the general use of properties “Regulations Governing the Use of Urban Properties”, the retailing industry is classified as an industry engaged in commercial activities and therefore a valid and accurate certificate proving the property ownership of the establishment intended for the retailing activities is required;
7. The applications must meet the stipulated requirements for licensing and the prescribed sanitary conditions required for establishments for import. The applicants can refer to the “Different Types of Guidelines” on the webpage of “Guidelines from the Department of Food and Animal Inspection and Control” (http://www.iacm.gov.mo/sis/) of the IACM for details;
8. Each application form is limited to application for one service item only;
9. Each application item requires submission of a completed Operation Proposal and Information Registration Form for Retail/Import Establishments;
10. For applicants who are non-Macao residents, according to the existing regulation “Regulation on Prohibition of Illegal Employment”, personal and direct engagement of non-Macao residents in activities for personal interests is subject to prior administrative approval;
11. For applications made by legal persons, if the shareholders or members of the administration and management body of a company are non-Macao residents who are responsible for the management of company and/or performing the business duties on behalf of the company, they must obtain relevant working permits;
12. In the case of necessity, the original copies of the supporting documents required by the administrative body should be submitted.

Renewal

Documents to be submitted1. Application Form for Retail License/Import Registration of Meat (Mod. 035/SIS);
2. A legible photocopy of the identity document of the license holder (with both sides of the identity document photocopied on the same page of an A4 sized paper);
3. The original import registration issued last year;
4. A photocopy of M/8 Form – Business Registration Tax Demand Note of the renewed year issued by the Financial Services Bureau recently.

Important notes1. After submission of all required documents, the establishment shall undergo inspection conducted by the relevant unit of the IACM;
2. The above-mentioned import registration requires renewal application, which is only approved when the stipulated requirements are met;
3. Import registrations issued are non-transferable and the registered establishments must not be moved;
4. The renewal procedure shall only be processed upon receipt of all required documents;
5. The import registration is valid till the end of the year in which it is issued. The license holders are required to file renewal application for import registration from January to February each year;
6. Though the import registration does not require payment of an annual license fee, the applicants are required to file their applications which shall be examined and assessed to determine if they meet the necessary requirements for registration of establishments for import;
7. For applicants who are non-Macao residents, according to the existing regulation “Regulation on Prohibition of Illegal Employment”, personal and direct engagement of non-Macao residents in activities for personal interests is subject to prior administrative approval;
8. For applications made by legal persons, if the shareholders or members of the administration and management body of a company are non-Macao residents who are responsible for the management of company and/or performing the business duties on behalf of the company, they must obtain relevant working permits.

Cancellation

Documents to be submitted1. The applicant should submit the original letter for cancellation of import registration application to the IACM;
2. The license holder should submit the original letter for cancellation of the issued import registration to the IACM;
3. A legible photocopy of the identity document of the applicant/license holder (with both sides of the identity document photocopied on the same page of an A4 sized paper);
4. A photocopy of M/1 Form for filing of business termination/cancellation of business registration in the Financial Services Bureau;
5. The original import registration issued last year.
