| Documents to be submitted | 1. Application form (Format Mod. 058/DLA/SAL); 2. Photocopy of Vehicle Registration Card; 3. Photocopy of the Ownership Registration Document of the vehicle; 4. Photocopy of Industrial Tax of the vehicle (if applicable); 5. A set of 4 photos of the advertisements to be affixed onto the body of the vehicle (front, back, left side and right side views of the vehicle. The photos must be able to show a clearly legible view of the layout of the advertisements and of the Vehicle License Plate No.); 6. The sketch or blueprint of the advertisement (the advertising content can be presented in various languages, but one of them must be the official languages of MSAR); 7. Photocopy of document(s) certifying registration of trademark (if applicable); 8. If the applicant is a natural person, a photocopy of the ID document of the signatory should be submitted; 9. If the applicant is a legal person, the application form should bear its legal representative’s signature, which must be notarized by a public notary office or a private notary; or alternatively, the signed application form may be submitted together with a photocopy of the applicant’s ID document as well as a photocopy of a valid Business Registration Certificate issued by the Commerce and Movable Property Registry; 10. In the cases where the contents of the billboard/signboard are related to advertisement of drugs, prescription drugs, prostheses, medical or paramedical treatment methods, or substances presented as having health benefits, the Letter of Approval of Advertisement issued by the Health Bureau should be submitted together with photocopies of the advertising script with the official stamp of the Bureau or other relevant supporting documents.
